Commission for a license to engage in business as a distrib-
utor, a producer-distributor, a distributing broker, or to
conduct a retail store as defined in this Act, or in any one
or more of said business.

(A) Such application shall be in writing, in
such form as the Commission may prescribe and shall
in every case include or be accompanied by the follow-
ing:

(a) The full name of the applicant, if an individual, the
names and addresses of all officers and directors, if a cor-
poration, or cooperative milk association, or the full name
of each member, if a firm.

(b) The post office address including street number if
any, at which the business is to be conducted.

(c) The market or markets in or in connection with
which the business of the applicant is proposed to be con-
ducted.

(d) Facts showing that the applicant has adequate per-
sonnel and facilities to properly conduct the business for
which a license is sought.

(e) A certificate from the health department of the
political sub-division in which the applicant desires to en-
gage in business to the effect that the applicant has com-
plied with and is complying with all applicable sanitary reg-
ulations of said health department; but the Commission
may require in lieu thereof or in addition thereto the cer-
tificate to the same effect of the State Department of
Health. Where the applicant proposes to engage in business
at more than one plant, or place, separate certificates shall
be furnished and may be required for each such plant or
place.

(f) A statement showing the number of delivery ve-
hicles which the applicant if granted a license will use.

(g) The license fee or fees required to be paid by this
Act.

(h) A schedule of wholesale and retail prices which the
applicant is paying and charging for each grade, quantity
and class of milk at wholesale and retail, respectively, and
in addition thereto the applicant shall in his application
consent to the jurisdiction of the Commission in establish-
ing wholesale and retail prices in the market or markets
specified in his application.

(G) In the case of each distributor such application
shall also include and be accompanied by the following:

(a) Either a bond or the cash equivalent in such
amounts as the Commission may prescribe with surety sat-
isfactory to the Commission conditioned for the payment of
