all obligations to producers within forty-five days after the
receipt of milk; and to secure the prompt payment of all
other sums properly required by the Commission to be paid;
or
(b) A financial statement showing evidence satisfactory
to the Commission to the effect that the applicant is of
sufficient financial responsibility to insure payment for
forty-five days' supply of milk, and to secure the other pay-
ments referred to in the preceding paragraph.
(c) A schedule of the prices now paid to producers from
whom milk is to be received for sale in each market by the
applicant.
(d) A statement as to the number and location of each
distributing plant which the applicant, if granted a license,
will operate; and if the applicant has been engaged in busi-
ness, a statement as to the monthly average number of
pounds of milk distributed at each such plant during, the
preceding year, or part thereof, during which he was so
engaged; and if the applicant has not been previously en-
gaged in business; an estimate as to such monthly average
to be so distributed during the period of the license applied
for.
(D) In the case of each person conducting a retail store
such application shall also include or be accompanied by a
statement as to the number and location of each retail store
which the applicant, if granted a license, will conduct.
(E) In addition to the matters and things required to
be included in or to accompany the application for license,
provided for by this section, each application shall set forth
such other facts and information as the Commission may
require in the form of application prescribed by it.
Section 6. (A) Upon receipt of any application for
license, or permit filed pursuant to this Act, the Commis-
sion shall examine the same and if it finds such application
to be in proper form, shall grant a license or permit as
the case may be as applied for subject to the provisions of
this section and of section 20 of this Act.
(B) The Commission may decline to grant a license,
or may condition and limit the granting of a license, or
may suspend or revoke a license, wither on its own mo-
tion or upon complaint filed with the Commission in writ-
ing by any interested person, when satisfied of the exist-
ence of any of the following facts:
(a) That the licensee purchasing milk from a producer
has without reasonable cause failed to account and make
