In the introductory language of subsection (b) of
this section, the reference to "any individual
who is applying on behalf of a corporation,
association, or government agency" is added to
provide for a corporate applicant. This addition
is patterned after § 19-319 of this title, which
provides for applicants for a license for a
hospital or related institution.

19-907. APPLICATIONS FOR LICENSE.

(A) IN GENERAL.

AN APPLICANT FOR A LICENSE SHALL:

(1)  SUBMIT AN APPLICATION TO THE SECRETARY; AND

(2)  PAY TO THE SECRETARY AN APPLICATION FEE OF
$10.

(B)  FORM AND CONTENTS.
THE APPLICATION:

(1)  SHALL BE ON THE FORM THAT THE SECRETARY
REQUIRES;

(2)  SHALL BE SIGNED AND VERIFIED BY THE
INDIVIDUAL; AND

(3)  SHALL INCLUDE:

(I)  THE NAME OF THE APPLICANT;

(II)  A STATEMENT THAT THE APPLICANT MEETS
THE REQUIREMENTS OF THIS SUBTITLE;

(III)  THE LOCATION OF THE PROPOSED
HOSPICE;

(IV)  THE NAME OF THE INDIVIDUAL WHO IS TO
BE THE MEDICAL DIRECTOR OF THE HOSPICE; AND

(V)  ANY OTHER INFORMATION THAT THE
SECRETARY REQUIRES.

(C)  REFUNDS PROHIBITED.

AN APPLICATION FEE MAY NOT BE REFUNDED.
