(2) Each report shall be on a form provided by
the Department and shall include: (i) the name and address
of the physician for whom the examination was performed;
(ii) the name, address, birth date, sex, political
subdivision, and place of employment of the person from whom
the specimen was taken; (iii) the date and results of the
examination based on the standard nomenclature of pathology
as compiled by the American College of Pathology; AND (IV)
SUCH OTHER INFORMATION AS THE SECRETARY MAY REQUIRE TO CARRY
OUT THE PROVISIONS OF THIS SECTION. THE PHYSICIAN SHALL
REPORT EACH CASE OF CANCER OR MALIGNANT DISEASE UNDER HIS
CARE TO THE DEPUTY HEALTH OFFICER OF THE COUNTY WHEREIN THE
PATIENT RESIDES.
(2) A PHYSICIAN'S REPORT SHALL BE ON A FORM
PROVIDED BY THE DEPARTMENT AND CONTAIN SUCH INFORMATION AS
REQUIRED BY THE SECRETARY TO CARRY OUT THE PROVISIONS OF
THIS SECTION.
(3) A DEPUTY HEALTH OFFICER RECEIVING ANY
PHYSICIAN'S REPORT UNDER THIS SUBSECTION SHALL PERIODICALLY
FORWARD THE REPORTS TO THE SECRETARY.
(4) INFORMATION MAY NOT BE RELEASED THAT
IDENTIFIES THE PATIENT.
(C) (1) UNLESS A PHYSICIAN SUBMITS TISSUE TO A
LABORATORY FOR DIAGNOSIS, UNDER SUBSECTION (B) OF THIS
SECTION, THE PHYSICIAN SHALL REPORT EACH CASE OF CANCER OR
OTHER MALIGNANT DISEASE UNDER HIS CARE TO THE DEPUTY HEALTH
OFFICER OF THE SANITARY DISTRICT WHEREIN THE PATIENT
RESIDES.
(2) A PHYSICIAN'S REPORT SHALL BE ON A FORM
PROVIDED BY THE DEPARTMENT AND CONTAIN SUCH INFORMATION AS
REQUIRED BY THE SECRETARY TO CARRY OUT THE PROVISIONS OF
THIS SECTION.
(3) A DEPUTY HEALTH OFFICER RECEIVING ANY
PHYSICIAN'S REPORT UNDER THIS SUBSECTION SHALL PERIODICALLY
FORWARD THE REPORTS TO THE SECRETARY.
[(3)] (D) (C) (1) The report of cancer OR OTHER
MALIGNANT DISEASE cases made by the Secretary pursuant to
[these provisions] THIS SECTION shall be in numerical form
and shall not disclose the identity of any persons to whom
they relate.
[(4)] (2) To determine compliance with this
section, the Secretary, health officer, or their respective
agents may inspect pertinent records of laboratories
PHYSICIANS subject to the provisions of this section.
(3) THE SECRETARY, WHEN FEASIBLE, SHALL CONDUCT
INVESTIGATIONS INTO THE CAUSE, MORTALITY, TREATMENT,
PREVENTION AND CURE OF CANCER AND OTHER MALIGNANT DISEASES.
