(5)  Impersonates another practitioner;

(6)  Is convicted of a crime involving moral
turpitude;

(7)  Habitually uses alcohol, or any narcotic or
stimulant to the extent that the individual is unable to
perform professional duties competently; [or]

(8)  Solicits or advertises in a false or
misleading manner or in any other manner not approved by the
Board[.];

(9)  IS IMMORAL UNETHICAL IN THE CONDUCT OF THE
PRACTICE OF CHIROPRACTIC;

(10)  IS PROFESSIONALLY, PHYSICALLY, OR MENTALLY
INCOMPETENT;

(11)  ABANDONS A PATIENT;

(12)  PROMOTE THE SALE OF MISREPRESENTS THE
EFFECTIVENESS OF ANY TREATMENT, DRUGS, DEVICES, APPLIANCES,
OR GOODS TO A PATIENT SO AS TO EXPLOIT THE PATIENT FOR
FINANCIAL GAIN;

(13)  WILLFULLY MAKES OR FILES A FALSE REPORT OR
RECORD IN THE PRACTICE OF CHIROPRACTIC;

(14)  WILLFULLY FAILS TO FILE OR RECORD ANY
MEDICAL REPORT AS REQUIRED BY LAW;

(15)  WILLFULLY IMPEDES OR OBSTRUCTS THE FILING
OR RECORDING OF ANY MEDICAL REPORT AS REQUIRED BY LAW;

(16) ON PROPER REQUEST, FAILS TO PROVIDE DETAILS
OF A PATIENT'S MEDICAL RECORD TO ANOTHER CHIROPRACTIC,
PHYSICIAN, OR HOSPITAL;

(17) SOLICITS PROFESSIONAL PATRONAGE THROUGH AN
AGENT OR OTHER PERSON OR PROFITS FROM THE ACTS OF A PERSON
WHO IS REPRESENTED AS AN AGENT OF THE CHIROPRACTOR;

(18) (16) PAYS OR AGREES TO PAY ANY SUM TO ANY
PERSON FOR BRINGING OR REFERRING A PATIENT;

(19) PRACTICES CHIROPRACTIC WITH AN UNAUTHORIZED
PERSON OR AIDS AN UNAUTHORIZED PERSON IN THE PRACTICE OF
CHIROPRACTIC;

(20) GROSSLY, WILLFULLY, AND CONTINUALLY:
