Ch. 909

THE APPLICANT SHALL BE AT LEAST 18 YEARS OLD.

(D)    EDUCATION.

(1)    AN APPLICANT FOR LICENSURE AS AN
OCCUPATIONAL THERAPIST SHALL:

(I)     HAVE SUCCESSFULLY COMPLETED THE
ACADEMIC REQUIREMENTS OF AN EDUCATIONAL PROGRAM IN
OCCUPATIONAL THERAPY RECOGNIZED BY THE BOARD AND ACCREDITED
BY THE AMERICAN MEDICAL ASSOCIATION IN COLLABORATION WITH
THE AMERICAN OCCUPATIONAL THERAPY ASSOCIATION. THE
EDUCATIONAL PROGRAM SHALL HAVE CONCENTRATION IN BIOLOGICAL
OR PHYSICAL SCIENCE, PSYCHOLOGY AND SOCIOLOGY, AND SELECTED
TREATMENT MODALITIES.

(II)   HAVE SUCCESSFULLY COMPLETED SIX
MONTHS OF SUPERVISED FULL-TIME FIELD WORK EXPERIENCE AT A
RECOGNIZED EDUCATIONAL INSTITUTION OR A TRAINING PROGRAM
APPROVED BY THE EDUCATIONAL INSTITUTION WHERE THE PERSON MET
THE ACADEMIC REQUIREMENTS.

(2)    AN APPLICANT FOR LICENSURE AS AN
OCCUPATIONAL THERAPY ASSISTANT SHALL:

(I)     HAVE SUCCESSFULLY COMPLETED THE
ACADEMIC REQUIREMENTS OF AN EDUCATIONAL PROGRAM FOR
OCCUPATIONAL THERAPY ASSISTANTS RECOGNIZED BY THE BOARD AND
APPROVED BY THE AMERICAN OCCUPATIONAL THERAPY ASSOCIATION.

(II)   HAVE SUCCESSFULLY COMPLETED A MINIMUM
OF TWO MONTHS FULL-TIME FIELD WORK EXPERIENCE AT A
RECOGNIZED EDUCATIONAL INSTITUTION OR A TRAINING PROGRAM
APPROVED BY THE EDUCATIONAL INSTITUTION WHERE THE PERSON MET
THE ACADEMIC REQUIREMENTS.

(E)    AN APPLICANT EXCEPT THOSE LICENSED UNDER SECTION
924 (B), AN APPLICANT WHO HAS PRACTICED AN AN AS AN
OCCUPATIONAL THERAPY ASSISTANT FOR FOUR YEARS WITH A MINIMUM
OF SIX MONTHS FULL-TIME SUPERVISED FIELD WORK EXPERIENCE MAY
TAKE THE EXAMINATION TO BE LICENSED AS AN OCCUPATIONAL
THERAPIST WITHOUT MEETING THE EDUCATIONAL REQUIREMENTS MADE
OTHERWISE APPLICABLE.

(F) THE APPLICANT SHALL MEET ANY OTHER QUALIFICATIONS
OR REQUIREMENTS THAT THE BOARD ESTABLISHES FOR LICENSE

APPLICANTS.

921. APPLICATIONS FOR LICENSES.

TO APPLY FOR A LICENSE, AN APPLICANT SHALL SUBMIT TO
THE BOARD:

(A)    AN APPLICATION ON THE FORM REQUIRED BY THE BOARD;

AND

(B)    THE APPLICATION FEE SET BY THE BOARD.
