424A 422A.

(1) Insurance under any group life insurance policy issued pur-
suant to Sections 420
through 424 SECTIONS 418 THROUGH 422
may, if seventy-five per centum of the then insured employees OR
MEMBERS OR ANY CLASS OR CLASSES THEREOF, elect, be
extended to insure the spouse and
MINOR minor children UNDER
THE AGE OF 21,
or any class or classes thereof, of each such insured
employee
OR MEMBER who so elects, in amounts in accordance with
a plan which precludes individual selection by the employees
OR
MEMBERS or by the employer or trustee. THE POLICY MAY PRO-
VIDE THAT THE TERM 'MINOR CHILDREN' SHALL INCLUDE
THE INSURED EMPLOYEE'S OR MEMBER'S CHILD UNDER 21
YEARS OF AGE OR HIS CHILD 21 YEARS OR OLDER WHO
IS ATTENDING AN EDUCATIONAL INSTITUTION AND RE-
LYING UPON THE INSURED EMPLOYEE OR MEMBER FOR
FINANCIAL SUPPORT. The insurance on the life of any one family
member
SPOUSE OR CHILD shall not be in excess of fifty per centum
of the insurance on the life of the insured employee or the amount
shown in the schedule below, whichever is less:

OR MEMBER, OR $2,000 IN THE CASE OF A SPOUSE, OR
$1,000 IN THE CASE OF A CHILD, WHICHEVER IS LESS.

Premiums for the insurance on such family members SPOUSE
OR CHILD shall be paid by the policyholder, either from the
employer's
POLICYHOLDER'S funds or funds contributed by him,
trustee's funds, or from funds contributed by the insured employees
OR MEMBERS, or from both.

(2)    A spouse OR DEPENDENT CHILD insured pursuant to this
section shall have the same conversion right as to the insurance on his
or her life as if vested in the employee.
BE ENTITLED TO THE
RIGHTS OF CONVERSION SET FORTH IN SECTION 434 IN
EVENT OF TERMINATION OF EMPLOYMENT OF THE EM-
PLOYEE OR IN EVENT OF TERMINATION OF MEMBERSHIP
IN THE CLASS OR CLASSES ELIGIBLE FOR COVERAGE UN-
DER THE POLICY, AND THE RIGHTS OF CONVERSION SET
FORTH IN SECTION 435 IN EVENT THE GROUP LIFE POLICY
TERMINATES OR IS AMENDED TO TERMINATE THE INSUR-
ANCE OF THE SPOUSE OR DEPENDENT CHILD.

(3)    Notwithstanding the provisions of Section 433 only one
certificate need be issued for each family unit if a statement con-
cerning any dependent's coverage is included in such certificate.

Sec. 2. And be it further enacted, That this Act shall take effect
July 1, 1969
