3.    On the day of the next presidential election, I shall be at least
twenty-one years of age and I have or will have by the time of such
next presidential election resided in this State in the ward or elec-
tion district in which I shall offer to vote at least forty-five days
next preceding such general presidential election, and I am now

residing in the ............ precinct of the ................ election district

in the County of........................................ (or in the................ precinct

of the ........ ward in the ........ Legislative District of the City of

Baltimore), State of Maryland.

4.    I believe I am entitled under the law of this State to vote at
the presidential election to be held November..................,19....

5.    I hereby make application for a presidential and vice-
presidential ballot. I have not voted and will not vote otherwise
than by this ballot at that election.

28-4. Same—Mailing duplicate to state of former residence.

The registrar shall immediately mail to the appropriate official of
the state in which the applicant last resided a duplicate of the
application.

28-5. Same—Filing and indexing duplicate received from another
state.

Every registrar shall file each duplicate application or other
official information received by him from another state indicating
that a former resident of Maryland has made application to vote
at a presidential election in another state and shall maintain an
alphabetical index thereof, for a period of six months after the
election.

28-6. Ballot given applicant.

If satisfied that the application is proper and that the applicant
is qualified to vote under this subtitle the registrar shall give the
applicant a ballot for presidential and vice-presidential electors.

28-7. Marking, folding and sealing ballot; certification.

(a)    The applicant upon receiving the ballot for presidential
and vice-presidential electors shall mark forthwith the ballot in the
presence of the registrar but in a manner that the registrar cannot
know how the ballot is marked. He shall then fold the ballot in the
presence of the registrar so as to conceal the markings, and deposit
it and seal it in an envelope furnished by the registrar.

(b)    The voter shall enclose the envelope containing the ballot
in a carrier envelope which shall be securely sealed. There shall
be imprinted on the outside of the carrier envelope a statement sub-
stantially as follows:
