any pecuniary interest, either direct or indirect, in the operation of
the machines sought to be licensed, have any pecuniary interest
either direct or indirect, in the operation of such machines in any
other establishment where such machines are operated; (10) the names
of all persons, firms or corporations holding any liens on the machines
to be licensed, whether oral or written, recorded or unrecorded; (11)
copies of all leases or rental agreements between the person applying
for licenses and the person, firm or corporation from whom the
premises where such machines are to be operated or leased; (12) the
names of all employees or other persons entitled to any share in the
profits to be derived from the machines sought to be licensed.; (13)
AND SUCH OTHER INFORMATION AS MAY BE PRESCRIBED
BY SAID COUNTY COMMISSIONERS.
At the time of application for a new license and/or renewal of
license, such application shall be accompanied by an inventory under
affidavit of machines licensed or to be licensed containing the fol-
lowing information:
(1) Date purchased
(2) Seller's name and address
(3) Description of machine
(4) Serial number
(5) Cost
(6) Lien, if any, and amount thereof and name and address of
lien holder.
[no] No applicant shall be eligible for the license provided in Sub-
section (b) above unless he shall [have been a bona fide resident of
Charles County for five years preceding the date of his application,
and who shall have been a registered voter of the last preceding
election, and] meet the resident requirements of Sub-section (b),
Section 84C, and unless said applicant shall state under oath that
no other person, firm or corporation is financially interested in the
business to be conducted by said licensee, and such additional informa-
tion as the Board of County Commissioners may from time to time
require on forms prescribed by said Board of County Commissioners
and filed with the said License Clerk.
Upon receipt of a properly executed application and upon receipt
of the proper license fee AND UPON APPROVAL OF THE BOARD
OF COUNTY COMMISSIONERS, the said Clerk shall [forthwith]
issue an appropriate certificate evidencing that such license is thereby
issued and the required license fees are thereby paid and said certifi-
cate shall enumerate and list a number for each of the devices thereby
licensed for said premises. At the time of issuing said certificate, the
Clerk shall also issue a metal plate which shall have stamped thereon a
number, corresponding to the number assigned to the device on the
certificate, which metal plate thereafter shall be securely fastened to
the amusement device in such a manner as will be visible. All ma-
chines or devices licensed hereunder shall be maintained and operated
on the inside of the licensed premises only.
(d) Each distributor securing a license hereunder shall on or before
the tenth day of each month report the name and address of every
