to the Department in Baltimore City upon a form furnished by the
Department and every said application shall be accompanied by a
fee of One ($1.00) Dollar.
(b) Every said application shall state the full name [(first, middle
and last names)], residence address, race, color, sex, height, weight
and date of birth of the applicant and shall state whether the applicant
has heretofore been licensed as an operator or chauffeur, and if so,
when and by what State or County, and whether any such license has
ever been suspended, revoked, cancelled or refused and, if so, the date
of and reason for such suspension, revocation, cancellation or refusal,
and every said application shall state the applicant's general physical
condition, occupation and other pertinent facts as the Department may
require. Every said application shall be signed by the applicant and
verified in person before a person authorized to administer oaths.
91. (Operating Fees) (a) Upon surrendering the instruction and
examination permit and undergoing a satisfactory examination as
to his/her qualifications, such person shall be entitled to receive the
license applied for upon the payment of the fees [hereinafter] pro-
vided for in sub-section (d) herein, [of this section.]

(d) The following fees shall be paid to the Department for the
issuance of the original licenses to operate motor vehicles, motor-
cycles, and bicycles with motor attachment.
Two ($2.00) Dollars to operate motor vehicles other than motor-
cycles and bicycles with motor attachments.
One ($1.00) Dollar to operate a motorcycle and such license shall
not be valid for the operation of any other type of vehicle.
One ($1.00) Dollar to operate a bicycle with motor attachment or
motor scooter and such license shall not be valid for the operation of
any other type of vehicle.
Three ($3.00) Dollars to operate a motor vehicle as a chauffeur as
that word is defined.
A chauffeur's license, unless suspended or revoked as hereinafter
provided, shall expire two (2) years from the date of issue and shall
be renewable on or before the date of expiration upon application on
forms furnished by the Department and upon payment of a fee of
Three ($3.00) Dollars.
Sec. 2. And be it further enacted, That any and all public general
or public local laws, or any part or parts thereof, inconsistent with
the provisions of this Act, be and they are hereby repealed to the
extent of any such inconsistency.
Sec. 3. And be it further enacted, That this Act shall take effect
July 1, 1957.
