Carroll Hospice. Provide a grant equal to the lesser of (i)
$500,000 or (ii) the amount of the matching fund provided, to

the Board of Trustees of Carroll Hospice, Inc. for the
construction of an inpatient facility for end of life patients,
located in Westminster. Notwithstanding the provisions of
Section 1(5) of this Act, the matching fund may include real
property (Carroll County)

Chase Brexton Health Services, Inc. Provide a grant equal to
the lesser of (i) $100,000 or (ii) the amount of the matching fund

provided, to the Board of Directors of Chase Brexton Health
Services, Inc. for the acquisition, renovation, design,
construction, and capital equipping of a building to increase
operating space, to house expanded dental and mental health
services, and to provide space for administration and

development offices, located in Baltimore City, subject to the
requirement that the grantee grant and convoy an historic
easement to the Maryland Historical Trust (Baltimore City)

Chelsea School. Provide a grant equal to the lesser of (i)
$300,000 or (ii) the amount of the matching fund provided, to
the Board of Directors of The Chelsea School, Inc. for the
planning, design, repair, renovation, reconstruction,
construction, and capital equipping of a classroom building and
gymnasium and for the demolition and lead abatement of the
annex of the Chelsea School located at 711 Pershing Drive in
Silver Spring. Notwithstanding the provisions of Section 1(5) of
this Act, the matching fund may include real property, in kind
contributions, or funds expended prior to the effective date of
this Act (Montgomery County)

Cheverly Community Center. Provide a grant in the total
principal amount of $100,000, to the Mayor and City Council of
the Town of Cheverly for the planning and design of an addition
to the Cheverly Community Center, located in Cheverly (Prince
George's County)

principal amount of $100,000, to the Board of Directors of
Community Crisis Services, Inc. for the acquisition, repair,
renovation, and capital equipping of a building, to be used as
the headquarters for a telephone based crisis center, located in
Hyattsville (Prince George's County

Community Services for Autistic Adults & Children. Provide a
grant equal to the lesser of (i) $300,000 or (ii) the amount of th
e
matching fund provided, to the Board of Directors of
Community Services for Autistic Adults and Children, Inc. for
the acquisition, planning, design, construction, and capital
equipping of an administration and training center for
