Ch. 22 1997 LAWS OF MARYLAND (E) THE SECRETARY MAY DETERMINE THAT LICENSES ISSUED UNDER THIS 11-404. (a) (1) To apply for a license, an applicant shall sign and submit to the (2) The application shall include: (i) The name of the applicant; (ii) The address at which the business is to be conducted, including, if (iii) The address of each branch, subsidiary, or affiliate that the (iv) If the applicant is a corporation, the date of its incorporation; (v) The name and residence address of each owner of or partner in the (vi) Any other pertinent information that the Commissioner requires. (b) With the application, the applicant shall pay to the Commissioner: (1) An investigation fee of $100; and (2) A license fee of [$125] $250. (c) (1) For each license for which an applicant applies, the applicant shall (i) A separate application; and (ii) Pay a separate investigation fee and license fee. (2) If an applicant applies for three or more licenses at the same time, the 11-409. (a) (1) A LICENSE ISSUED ON OR BEFORE SEPTEMBER 30, 1997 EXPIRES ON (2) A license ISSUED ON OR AFTER OCTOBER 1, 1997 expires on December (b) Before a license expires, the licensee may apply for a new license.
