(1)     COMPLEXITY OF TESTING THAT THE LABORATORY MAY OFFER OR
PERFORM;

(2)     CLASSES OF SERVICES THAT MAY BE OFFERED;

(3)     TESTS OR EXAMINATIONS THAT MAY BE OFFERED OR PERFORMED
BY THE LABORATORY; OR

(4)     ANY COMBINATION OF (1), (2), AND (3).

(D)    A MEDICAL LABORATORY MAY NOT OPERATE IN ANY MANNER BEYOND
WHAT IS DESIGNATED BY ITS LICENSE.

(E)     A MEDICAL LABORATORY LICENSE ISSUED BY THE SECRETARY UNDER
THIS SUBTITLE IS NOT TRANSFERRABLE.

While it is effective, a permit authorizes the permit holder:

(1)     (i) To operate or to represent or service the medical laboratory
named in the permit; or

(ii) To perform tests or examinations pertaining to individuals in this
State at a medical laboratory located outside this State; and

(2)     To offer the classes of services set forth in the permit.]

(a) A [permit] LICENSE expires on the [first anniversary of its effective date,]
DATE SET BY THE SECRETARY, unless the [permit] LICENSE is renewed [for a 1-year
term] as provided in this section.

[(b) At least 1 month before the permit expires, the Department shall send to the
permit holder, by first class mail to the last known address of the holder, a renewal notice
that states:

(1)     The date on which the current permit expires;

(2)     The date by which the renewal application must be received by the
Secretary for the renewal to be issued and mailed before the permit expires; and

(3)     The amount of the renewal fee.]

[(c)](B) Before the [permit] LICENSE expires, the [permit] LICENSEE [holder
periodically] may renew [it] ITS LICENSE for an additional [1-year] term, if the
[permit] LICENSEE [holder]:

(1)     Otherwise is entitled to the [permit] LICENSE;

(2)     Pays to the Department the renewal fee set by the Secretary AND ANY
OUTSTANDING LICENSING OR PROFICIENCY TESTING FEES; and

(3)     Submits to the Secretary:
