serviced] and its [director] PERSONNEL meet the STANDARDS AND requirements
[that] OF THE SUBTITLE AND IN REGULATIONS ADOPTED BY the Secretary [adopts
under] PURSUANT TO this subtitle.

17-207.

(a)     An applicant for a [permit] LICENSE shall submit an application to the
Secretary on the form that the Secretary requires.

(b)     [(1)]An application for a [permit] LICENSE to operate a medical laboratory
shall include:

[(i)] (1) The name of the owner;

[(ii)](2) The classes of services, COMPLEXITY OF TESTING, OR THE
TESTS OR EXAMINATIONS that the medical laboratory would provide; and

[(iii)] (3) Any other information that the Secretary requires.

[(2) An application for a permit to represent or service a medical laboratory
shall include satisfactory evidence that the medical laboratory to be represented or
serviced and its director meet the requirements that the Secretary adopts under this
subtitle.

(3) In addition to the requirements of paragraph (1) of this subsection, an
application for a permit to perform tests or examinations pertaining to individuals in this
State at a medical laboratory located outside this State shall include satisfactory evidence
that the medical laboratory and its director meet the requirements that the Secretary
adopts under this subtitle or the standards under the Federal Clinical Laboratory
Improvement Amendments of 1988.]

(c)     The applicant shall pay to the Department the application fee set by the
Secretary.

(a)     The Secretary shall issue a [permit] LICENSE to any applicant who meets the
STANDARDS AND requirements of this subtitle AND IN REGULATIONS ADOPTED
PURSUANT TO THIS SUBTITLE.

(b)     [The Secretary shall include on each permit that the Secretary issues] A
MEDICAL LABORATORY LICENSE SHALL INCLUDE THE NAME OF THE:

(1)     [The name of the medical] MEDICAL laboratory;

(2)     [The name of its] LABORATORY director; AND

(3)      [The name of its owner] OWNER OF THE LABORATORY [; and

(4)     The classes of services that the medical laboratory may offer].
(C) A MEDICAL LABORATORY LICENSE SHALL DESIGNATE THE:
