(b) An applicant for a license as a sports agent shall
submit a written application to the Secretary on a form that the
Secretary provides, which shall include the following
information:

(4) (i) The names and addresses of all persons,
except bona fide employees on fixed salaries, who are financially
interested as partners, associates, or profit [shares] SHARERS in
the operation of the business of the sports agent; or

(d) A license issued under this [act] SUBTITLE is valid for
1 year from the date of issuance.
