$40 per week. The maintenance so provided shall be at the
expense of the employer and insurer.
[(e)] (G) Transportation costs to and from the place
of training are not reimbursable to a claimant residing at home
during vocational rehabilitation training, except that the
Commission may allow reasonable transportation costs in unusual
cases.
[(f)] (H) If an employee refuses to accept vocational
rehabilitation [training,] TRAINING pursuant to an order of the
[Commission,] COMMISSION and the Commission determines the
refusal to be unreasonable, all payments of compensation for
temporary total disability shall be forfeited during the period
of refusal.
[(g)] (I) This subsection shall apply only to
injuries occurring on or after July 1, 1968.
(J) ALL INSURERS OR SELF-INSURERS SHALL SUBMIT TO THE
COMMISSION VOCATIONAL REHABILITATION PROGRESS REPORTS USING FORMS
THE REHABILITATION PROGRESS REPORT FORM AS PRESCRIBED BY THE
COMMISSION PERTAINING TO ANY EMPLOYEE WHO HAS RECEIVED TEMPORARY
TOTAL DISABILITY BENEFITS FOR A CONTINUOUS PERIOD OF 6 MONTHS.
THEREAFTER, INSURERS AND SELF-INSURERS SHALL PROVIDE THE
COMMISSION WITH FURTHER REHABILITATION PROGRESS REPORTS AT
120-DAY INTERVALS OR SOONER AS REQUESTED BY THE COMMISSION. IF
THE COMMISSION FINDS THAT ANY INSURER OR SELF-INSURER HAS FAILED
TO FILE SUCH RECORDS WITHIN THE TIME PRESCRIBED BY THIS
SUBSECTION, THE COMMISSION MAY ASSESS AGAINST THE PARTY A FINE
NOT TO EXCEED $500.

(A) ON OR BEFORE JULY 1, 1988, THE COMMISSION SHALL ADOPT
GUIDES TO BE USED BY PHYSICIANS TO MEASURE AND REPORT TO THE
COMMISSION ALL MEDICAL EVALUATIONS OF PERMANENT IMPAIRMENTS.
(B) THE GUIDES TO BE ADOPTED BY THE COMMISSION UNDER
SUBSECTION (A) ARE REGULATIONS UNDER SUBTITLE 1, TITLE 10 OF THE
STATE GOVERNMENT ARTICLE.
(C) ON OR AFTER JULY 1, 1987 AND UNTIL THE COMMISSION
ADOPTS GUIDES UNDER SUBSECTION (A) OF THIS SECTION, PHYSICIANS
SHALL USE THE AMERICAN MEDICAL ASSOCIATION'S "GUIDES TO THE
EVALUATION OF PERMANENT IMPAIRMENT", AS AMENDED, TO MEASURE ALL
MEDICAL EVALUATIONS OF PERMANENT IMPAIRMENTS. PHYSICIANS SHALL
REPORT THEIR FINDINGS IN ACCORDANCE WITH THOSE GUIDELINES.
(D) UNTIL THE COMMISSION ADOPTS GUIDES UNDER SUBSECTION (A)
OF THIS SECTION, A PHYSICIAN, IN EVALUATING AN IMPAIRMENT, MAY
SUBMIT TO THE COMMISSION ADDITIONAL INFORMATION, INCLUDING.
