In addition to any other requirement of law, each container
of controlled poison shall bear a plainly written label that
includes:

(1)  The word "POISON";

(2)  The name of the substance;

(3)  The date of sale; and

(4)  The name and business address of the seller.]
[5-115.

(a)  This section does not apply to the dispensing of a
controlled poison on the prescription of an authorized prescriber
if the quantity or dose is not unusual.

(b)  A person who sells, delivers, or provides any
controlled poison shall record in a poison register:

(1)  The date of sale or delivery;

(2)  The name and address of the person for whom the
controlled poison is provided;

(3)  The name and quantity of the controlled poison;

(4)  The purpose for which the controlled poison is to

be used;

(5) The name of the dispenser; and

(6) The signature of the individual who receives the
controlled poison.

(c)  If a controlled poison is sold, delivered, or provided
to an individual under the age of 15 years, the written order
required by § 5-113(c) of this subtitle shall be inserted in the
poison register instead of the entries otherwise required by
subsection (b) of this section.

(d)  A poison register shall be:

(1)  In the form required by the Board;

(2)  Kept for at least 2 years after the last entry;

and

(3) Open to inspection by any proper authority.

(e) Any person who is required to keep a poison register
shall display conspicuously in each store in which the person
sells controlled poison a sign that:
