(5) The signature of the Board.

(c) The Board shall issue a new certificate to replace a
lost, destroyed, or mutilated certificate if the certificate
holder pays a fee [of not more than $25] that is set by the
Board.

[(a)] Certification as a registered [sanitarian]
ENVIRONMENTAL HEALTH PRACTITIONER SANITARIAN authorizes the

registered [sanitarian] ENVIRONMENTAL HEALTH PRACTITIONER

SANITARIAN to use the title ["registered sanitarian"]
"ENVIRONMENTAL HEALTH PRACTITIONER SANITARIAN" and the initials

["R.S."] "R.E.H.P." after the name of the registered

sanitarian while the certification is effective.

[(b) Certification as a sanitarian in training authorizes
the sanitarian in training to use the title "sanitarian in
training" while the certification is effective.]

(a)  A certification as a registered [sanitarian]
ENVIRONMENTAL HEALTH PRACTITIONER SANITARIAN expires on the July
1 after its effective date, unless it is renewed for a [1] 2-year
term as provided in this section.

(b)  At least 1 month before the certification as a

registered [sanitarian] ENVIRONMENTAL HEALTH PRACTITIONER

SANITARIAN expires, the Board shall send to the registered

[sanitarian] ENVIRONMENTAL HEALTH PRACTITIONER SANITARIAN

certified, by first class mail to the last known address of the

registered [sanitarian] ENVIRONMENTAL HEALTH PRACTITIONER

SANITARIAN, a renewal notice that states:

(1)  The date on which the current certification
expires;

(2)  The date by which the renewal application must be
received by the Board for the renewal to be issued and mailed
before the certification expires; and

(3)  The amount of the renewal fee; AND

(4)  HOURS OF APPROVED TRAINING REQUIRED FOR RENEWAL
OF CERTIFICATION.

(c)  Before the certification of a registered [sanitarian]
ENVIRONMENTAL HEALTH PRACTITIONER SANITARIAN expires, the

registered [sanitarian] ENVIRONMENTAL HEALTH PRACTITIONER

SANITARIAN certified may renew it for an additional [1] 2-year
term, if the registered [sanitarian] ENVIRONMENTAL HEALTH
PRACTITIONER SANITARIAN:
