the nature and amount of the consideration received in exchange
therefor and the identity, of the person to whom the interest was
transferred; and (4) With respect to any interest acquired during the
year for which the statement is filed: (i) The date when, the manner in which, and the
identity of the person from whom the interest was acquired; and (ii) The nature and the amount of the
consideration given in exchange for the interest or, if acquired
other than by purchase, the fair market value of the interest at
the time acquired. (d) A schedule of each gift permitted by law in excess of
$25 in value or a series of gifts totaling $100 or more from any
one person received at any time during the year for which the
statement is filed by the person making the statement, or by any
other person at the direction of the person making the statement,
from, or on behalf of, directly or indirectly, any person who
does business with the State or is regulated by the State, or is
a registrant; provided, however, that neither gifts received from
members of the immediate family, other children, and parents of
the person making the statement, nor campaign contributions
regulated in accordance with the provisions of Article 33, [§]§
26-1 et seq. of this Code, or any other provision of State law
regulating the conduct of elections or the receipt of political
campaign contributions need be disclosed. This schedule, as to
each such gift, shall include: (1)  The nature and value of the gift; and (2)  The identity of the person from whom, or on
behalf of whom, directly or indirectly, the gift was received. 6-203. (a) (7) The executive director of the Commission shall
provide the forms for use in filing the statements set forth in
paragraph (2) OF THIS SUBSECTION, and shall make such forms
available in the office of the executive director of the
Commission as well as provide sufficient copies of such forms to
the chief administrative officers of Montgomery and Prince
George's counties for use by applicants and commissioners. The
forms provided shall, in all respects, replicate the provisions
of the forms provided pursuant to § [2-102(c)] 2-103(C) OF THIS
ARTICLE, excepting those modifications required pursuant to
paragraph (2) of this subsection. Article 41 - Governor - Executive and Administrative Departments 14E.
