18-212. HEALTH OFFICER RECORDS.

(A)  RECORD REQUIRED.

THE HEALTH OFFICER SHALL KEEP A RECORD OF EACH REPORT
THAT IS SUBMITTED TO THE HEALTH OFFICER UNDER § 18-201 OF
THIS SUBTITLE.

(B)  FORM AND CONTENTS.
THE RECORD SHALL:

(1)  BE ON THE FORM THAT THE SECRETARY PROVIDES;

(2)  STATE THE DATE OF THE REPORT;

(3)  STATE THE NAME OF THE PHYSICIAN WHO SUBMITS
THE REPORT;

(4)   STATE THE DISEASE IDENTIFIED IN THE REPORT;

(5)  STATE THE NAME AND RESIDENCE ADDRESS OF THE
PATIENT NAMED IN THE REPORT; AND

(6)   STATE THE ACTIONS TAKEN TO PREVENT THE
SPREAD OF THE DISEASE.

REVISOR'S NOTE: Subsections (a) and (b)(2) through (6)
of this section are new language derived without
substantive change from the first sentence of
former Article 43, § 79,

Subsection (b)(1) of this section is new language
derived without substantive change from the third
sentence of former Article 43, § 79 and the fifth
sentence of § 80.

In subsection (a) of this section, the reference
to a report under "§ 18-201 of this subtitle" —
which is derived from former Article 43, § 78 —
is substituted for the reference to reports "in
pursuance of §§ 77 and 78," in light of the
deletion of former Article 43, § 77. See
revisor's note to § 18-210 of this subtitle.

In subsection (b)(5) of this section, the term
"residence address" is substituted for the phrase
"localities in which they live", for clarity and
conformity to the terminology in the referenced §
18-201 of this subtitle.

In subsection (b)(6) of this section, the phrase
"actions taken to prevent the spread of the
disease" is substituted for the phrase
