(B)  CONTENTS.

AN APPLICATION SHALL INCLUDE:

(1)  THE NAME AND ADDRESS OF THE OWNER;

(2)  THE CLASSES OF TESTS THAT THE MEDICAL TEST
UNIT WOULD PROVIDE; AND

(3)  ANY OTHER INFORMATION THAT THE SECRETARY
REQUIRES.

(C)  FEES.

THE APPLICANT SHALL PAY TO THE DEPARTMENT THE
APPLICATION FEE SET BY THE SECRETARY.

REVISOR'S NOTE: This section is new language derived
without substantive change from the first
sentence and, as it related to fees, the second
sentence of former Article 43, § 54K(c)(l) and
the first clause of the second sentence of (e).

In subsection (a) of this section, the phrase "on
the form that the Secretary requires" is standard
language added to state expressly a provision
that only was implied in the former law: i.e.,
that applications may be made only on the form
required by the Secretary.

The second clause of former Article 43, § 54K(e),
which provided for disposition of the fees "in
the general funds of the State", is deleted as
unnecessary in light of § 2-104(j) of this
article, to the same effect.

see § 17-403 of this subtitle.

17-407. - ISSUANCE AND CONTENTS OF PERMIT.

(A)  ISSUANCE.
