9-612. PETITION.

(A)  IN GENERAL.

AN INDIVIDUAL WHO BELIEVES HIMSELF OR HERSELF TO BE A
DRUG ABUSER MAY FILE A PETITION TO BE COMMITTED TO THE
ADMINISTRATION, AS PROVIDED IN PART II OF THIS SUBTITLE.

(B)  JURISDICTION AND VENUE.

A PETITION UNDER THIS SECTION MAY BE FILED WITH THE
DISTRICT COURT IN OR CIRCUIT COURT FOR THE COUNTY WHERE THE
INDIVIDUAL RESIDES OR OTHERWISE MAY BE FOUND.

(C)  PETITION.

A PETITION UNDER THIS SECTION SHALL:

(1)  BE SIGNED AND VERIFIED BY THE PETITIONER;

(2)  STATE THE PETITIONER'S:

(I)  NAME;

(II)  ADDRESS;

(III)  TELEPHONE NUMBER, IF ANY;

(IV)  BIRTH DATE;

(V)  BIRTH PLACE;

(VI)  AGE;

(VII)  SEX;

(VIII)  MARITAL STATUS;

(IX)  OCCUPATION; AND

(X)  PHYSICAL DESCRIPTION;

(3)  CONTAIN STATEMENTS THAT SUPPORT THE BELIEF
THAT THE PETITIONER IS A DRUG ABUSER; AND
