(iii) So suffers from lung disease that
his forced expiratory volume, one second, when measured by
spirometry, is less than one liter;
(iv) Has a permanent impairment of both
eyes so that:
(A) The central visual acuity is
20/200 or less in the better eye, with corrective glasses;
or
(B) There is a field defect in which
the peripheral field has contracted to such an extent that
the widest diameter of visual field subtends an angular
distance no greater than 20 degrees in the better eye; OR
(v) Has a physical disability, as
certified by a licensed physician and determined by the
Administration, which is permanent, which substantially
impairs the person's mobility, and which is so severe that
the person would endure a hardship or be subject to a risk
of injury if the privileges accorded a person for whom a
vehicle is specially registered under this section were
denied[; or
(vi) Has a physical disability, as
certified by a licensed physician and determined by the
Administration, which is not permanent, but which
substantially impairs the person's mobility for no less than
5 weeks, and which is so severe that the person would endure
a hardship or be subject to a risk of injury if the
privileges accorded a person for whom a vehicle is specially
registered under this section were denied].
(2) This section applies only as to:
(i) A Class A (passenger) vehicle; or
(ii) A Class E (truck) vehicle with a 3/4
ton or less manufacturer's rated capacity and specially
equipped for the transportation of or operation by a
handicapped individual.
(b) Special registration may be issued under this
section only if the applicant submits:
(1) Proof satisfactory to the Administration
that the applicant, the dependent of the applicant, or the
individual who depends on the applicant for transportation,
as the case may be, is handicapped in the manner described
in subsection (a)(1) of this section; and
(2) As to an applicant on whom a handicapped
individual depends for transportation, a certification of
