Ch. 350

(1)  Fraudulently or deceptively obtains or
attempts to obtain a license for the applicant or licensee
or for another;

(2)  Fraudulently or deceptively uses a license;

(3)  Is convicted of a crime involving moral
turpitude;

(4)  Abandons a patient;

(5)  Is addicted to any narcotic or habitually is
intoxicated;

(6)  Provides optometric services while
intoxicated or under the influence of a drug;

(7)  Promotes the sale of devices, appliances, or
goods to a patient so as to exploit the patient for
financial gain;

(8)  Willfully makes or files a false report or
record in the practice of optometry;

(9)  Willfully fails to file or record any
optometric report as required by law;

(10)  Willfully impedes or obstructs the filing
or recording of any optometric report as required by law;

(11)  Willfully induces another to fail to file
or record any optometric report as required by law;

(12)  Fails to provide details of the optometric
record of a patient to a physician or another optometrist on
proper request by the patient;

(13)  Employs another person as a solicitor of
business;

(14)  Splits or agrees to split a fee for
optometric services with any person for bringing or
referring a patient;

(15)  Makes a willful misrepresentation in
treatment;

(16)  Aids an unauthorized person in the practice
of optometry;

(17)  Grossly and willfully:

(i) Overcharges for optometric services;
