TERM AND RENEWAL OF LICENSES. (A) TERM OF LICENSE. A LICENSE EXPIRES ON THE OCTOBER 1 AFTER ITS EFFECTIVE (B) RENEWAL NOTICE. AT LEAST 1 MONTH BEFORE THE LICENSE EXPIRES, THE BOARD (1) THE DATE ON WHICH THE CURRENT LICENSE (2) THE DATE BY WHICH THE RENEWAL APPLICATION (3) THE AMOUNT OF THE RENEWAL FEE. (C) APPLICATIONS FOR RENEWAL. BEFORE THE LICENSE EXPIRES, THE LICENSEE PERIODICALLY (1) OTHERWISE IS ENTITLED TO BE LICENSED; AND (2) PAYS TO THE BOARD A RENEWAL FEE OF $5. (D) APPLICATION FORM. THE RENEWAL APPLICATION FORM PROVIDED BY THE BOARD (1) NAME;
