THE RENEWAL APPLICATION FORM PROVIDED BY THE BOARD
SHALL REQUIRE THE LICENSEE TO STATE:

(1)  NAME;

(2)  LICENSE NUMBER;

(3)  DEGREE;

(4)  SCHOOL THAT AWARDED THE DEGREE;

(5)  YEAR OF GRADUATION;

(6)  PRINCIPAL OFFICE ADDRESS;

(7)  PRINCIPAL OFFICE TELEPHONE NUMBER; AND

(8)  WHETHER OR NOT THE LICENSEE IS IN ACTIVE
PRACTICE.

(E)  ISSUANCE OF RENEWAL.

THE BOARD SHALL RENEW THE LICENSE OF AND ISSUE A
RENEWAL CERTIFICATE TO EACH LICENSEE WHO MEETS THE
REQUIREMENTS OF THIS SECTION.

(F)  SURRENDER OF TEACHER'S LICENSE TO PRACTICE
DENTISTRY.

IF A TEACHER'S LICENSE TO PRACTICE DENTISTRY EXPIRES
BECAUSE THE LICENSEE NO LONGER IS A FULL-TIME FACULTY MEMBER
AT THE INSTITUTION NAMED ON THE LICENSE, THE LICENSEE SHALL
SURRENDER THE LICENSE TO THE BOARD SECRETARY WITHIN 30 DAYS.

REVISOR'S NOTE: This section is new language derived
without substantive change from the first clause
of the second sentence of Art. 32, § 5(c) and the
second sentence of (e)(2), from Art. 32, §§
5(e)(3) and 6(a) — except the last phrase of
that subsection, which now appears in § 4-312 of
this subtitle — and from Art. 32, §§ 6A(a),
28A(c), 29(d), 30(a), and the first clause of
32(b).

Subsection (b) of this section — which provides
for renewal notice — is new language added to
conform to current Board practice and to similar
provisions governing other health occupations in
present Art. 43. See, e.g., present Art. 43, §
634(c), as to psychologists, and present Art. 43,
§ 868(d), as to social workers.

In subsection (d)(6) and (7) of this section, the
term "principal" is added for clarity and to
conform to practice.
