(2) PAY TO THE BOARD AN APPLICATION FEE:

(I)  OF $50 FOR A GENERAL LICENSE TO
PRACTICE DENTISTRY OR A TEACHER'S LICENSE TO PRACTICE
DENTISTRY;

(II)  OF $15 FOR A LIMITED LICENSE TO
PRACTICE DENTISTRY;

(III)  OF $25 FOR A GENERAL LICENSE TO
PRACTICE DENTAL HYGIENE; OR

(IV)  SET BY THE BOARD FOR A TEACHER'S
LICENSE TO PRACTICE DENTAL HYGIENE.

(B)  ADDITIONAL REQUIREMENTS -- LIMITED LICENSE TO
PRACTICE DENTISTRY.

IN ADDITION TO THE OTHER REQUIREMENTS OF THIS SECTION,
AN APPLICANT FOR A LIMITED LICENSE TO PRACTICE DENTISTRY
SHALL SUBMIT TO THE BOARD A WRITTEN REQUEST, FROM THE
HOSPITAL, SANATORIUM, OR DENTAL SCHOOL TO WHICH THE PRACTICE
IS TO BE LIMITED, THAT THE LICENSE BE ISSUED.

(C)  ADDITIONAL REQUIREMENTS -- LICENSE TO PRACTICE
DENTAL HYGIENE.

(1)  AT LEAST 30 DAYS BEFORE THE EXAMINATION, AN
APPLICANT FOR A GENERAL LICENSE TO PRACTICE DENTAL HYGIENE
SHALL SUBMIT THE APPLICATION TO THE BOARD.

(2)  IN ADDITION TO THE OTHER REQUIREMENTS OF
THIS SECTION, AN APPLICANT FOR A LICENSE TO PRACTICE DENTAL
HYGIENE SHALL SUBMIT TO THE BOARD:

(I)  SATISFACTORY EVIDENCE OF GRADUATION
FROM AN APPROVED SCHOOL FOR DENTAL HYGIENISTS; AND

(II)  ANY OTHER CREDENTIALS THAT THE BOARD
REQUIRES.

REVISOR'S NOTE: This section is new language derived
without substantive change from the first and
third sentences of Art. 32, § 5(c) and the first
clause of the first sentence of (e)(1) and the
first clause of (3), from Art. 32, § 8, the
second and third sentences of § 27, and the first
sentence of § 28, and from Art. 32, § 28A(a)(4)
and (5).

Subsection (a)(1) of this section is derived in
part from the reference in present Art. 32, § 27
to an application form "prescribed and furnished
by the Board" and from the reference in present
Art. 32, § 5(e)(1) to an "appropriate
application". This provision is revised in
