Ch. 210

(1)  The name of the organization and a list of
the names and addresses of its officers and directors;

(2)  A full and complete statement of the
purposes and objectives of the organization and the purposes
for which the proceeds from a bingo operation will be used;

(3)  A statement under affidavit by the president
and treasurer of the organization or by its chief executive
and fiscal officer that (i) no agreement exists for the
diversion of any portion of the proceeds of a bingo
operation to any other person, partnership, or corporation,
and (ii) no person, partnership, organization, or
corporation shall receive any portion of the proceeds of the
bingo operation except in furtherance of the purpose of the
organization;

(4)  Any additional information the department of
(environmental protection] HEALTH may require.

(c)  An annual bingo license for the operation of the
game of bingo known as an "annual bingo license" shall
authorize the holder to operate a game of bingo at a
specified fixed location at any time during the year for
which the license is issued but not to exceed twice in any
one week. The applicant shall pay an annual license fee as
determined by the county department of [environmental
protection] HEALTH.

(d)  A temporary license for the operation of the game
of bingo known as a "ten-day bingo license" shall authorize
the holder to operate the game of bingo at a specified fixed
location for not more than ten days in any one year. The
applicant is limited to one license per calendar year. The
applicant shall pay a license fee as determined by the
county department of [environmental protection] HEALTH.

(e)  A one-day license for the operation of the game of
bingo known as a "one-day bingo license" shall authorize the
holder to operate the game of bingo at a specified fixed
location for not more than one day. The applicant shall pay
a license fee to be determined by the county department of
[environmental protection] HEALTH. An applicant may not be
granted more than three one-day licenses in any calendar
year.

(f)  Alcoholic beverages may not be sold or consumed in
the room in which a bingo game is being held, either during
the game or during any intermission between games.

(g)  The department of [environmental protection]
HEALTH of Montgomery County shall adopt regulations for the
conduct of the game of bingo and establish license fees
based upon the administrative costs of regulation and
issuance of each class of license in Montgomery County.
Cash prizes, not to exceed $500 per game, may be awarded in
