(4)  SUBMITS TO THE BOARD AN APPLICATION ON A
FORM REQUIRED BY THE BOARD; AND

(5)  PAYS THE BOARD THE LIMITED LICENSE FEE SET
BY THE BOARD.

(B)  A LIMITED LICENSE AUTHORIZES THE HOLDER TO:

(1)  TEACH DENTAL HYGIENE ONLY WITH THE DENTAL
SCHOOL DESIGNATED ON THE LICENSE; AND

(2)  PRACTICE DENTAL HYGIENE UNDER THE
SUPERVISION OF A LICENSED DENTIST WITH THE INSTITUTION
DESIGNATED ON THE LICENSE.

(C)  A LIMITED LICENSE AUTOMATICALLY SHALL BE VOID WHEN
THE LICENSEE STOPS PROVIDING THE SERVICES THAT ARE REQUIRED
FOR ISSUANCE OF A LICENSE UNDER THIS SECTION.

SECTION 2. AND BE IT FURTHER ENACTED, That this Act
shall take effect July 1, 1980.
