(2) If the applicant is an individual, his name
and address; if a partnership, the names and addresses of
all partners; if a joint venture, the names and addresses of
the parties to [it, if] IT; IF a corporation, the names and
addresses of all officers.

Article 59 - Mental Hygiene

11.

(b) (2) On or after July 1, 1977, persons 65 years
of age and older may be involuntarily or voluntarily
admitted to a facility only if their admission is
recommended by a geriatric evaluation unit. However, any
person admitted to a facility pursuant to the provisions of
§ 22 of this article may be admitted without his admission
HAVING BEEN recommended by a geriatric evaluation unit.
When an emergency admission of a person 65 years of age or
older has taken place, the geriatric evaluation unit of the
political subdivision of the person's residence shall be
informed on the first working day thereafter.

Article 59A - Mental Retardation

20A.

A private group home may not be established until a
certificate of approval has been obtained from the [director
Every] DIRECTOR. EVERY application for a certificate of
approval of a private group home shall be filed with the
Administration, on a form prescribed by the director, sworn
to by the applicant, and shall contain the following
information:

(1)  The name and address of the applicant;

(2)  The particular location for which the
certificate of approval is required, designating the street
number if practical, and if not practical then any
description which definitely locates the premises;

(3)  The name of the owner of the property upon
which the private group home is proposed, if other than the
applicant;

(4)  A statement that the applicant will conform
to all laws and regulations relating to the establishment,
maintenance, and operation of the private group home; and

(5)  A statement that the applicant has
sufficient financial resources or that sufficient financial
resources are available to the applicant for the
establishment and operation of the private group home.

SECTION 2. AND BE IT FURTHER ENACTED, That this Act
shall take effect July 1, 1979.
