(ii) That no cash prizes were offered,
except as authorized in paragraph (b)(3).

(iii) The amount and disposition of the
cash proceeds of the licensed activities.

(c)   The license shall be issued under the following
conditions:

(1)   The Sheriff shall charge a license fee of $1
for the issuance of each bingo license and a license fee of
$5 for each [carnival] PADDLE WHEEL LICENSE and EACH raffle
license.

(2)   [The Sheriff may not issue to a single
organization or auxiliary unit more than 25 licenses for
bingo games, four licenses for raffles, or two licenses for
carnivals in a calendar year. A licensee may not conduct
more than one bingo session per week.]UPON REQUEST AND
AUTHORIZATION, A LICENSE TO OPERATE PADDLE WHEELS ALSO SHALL
OPERATE AS A LICENSE TO CONDUCT BINGO GAMES.

(3)   [A license is required for each day a bingo
game is operated, unless the bingo game is held in a duly
licensed carnival.]

(I)   THE SHERIFF MAY NOT   ISSUE TO A SINGLE
ORGANIZATION MORE THAN 52 LICENSES FOR      BINGO GAMES, 12
LICENSES FOR RAFFLES, AND EIGHT LICENSES    FOR PADDLE WHEELS
WITHIN 1 CALENDAR YEAR.

(II)   EXCEPT AS PROVIDED IN PARAGRAPH (4),
NOT MORE THAN ONE BINGO LICENSE OR PADDLE WHEEL LICENSE
SHALL BE ISSUED TO A SINGLE ORGANIZATION IN A SINGLE
CALENDAR WEEK.

(4)   LICENSES ISSUED UNDER THIS SECTION TO
OPERATE PADDLE WHEELS OR TO CONDUCT BINGO GAMES SHALL BE
VALID FOR A PERIOD OF 24 CONSECUTIVE HOURS. HOWEVER, IF THE
LICENSE IS ISSUED FOR USE IN CONJUNCTION WITH AND AT THE
LOCATION OF A CARNIVAL, IT SHALL BE VALID DURING CARNIVAL
HOURS AND FOR THE DURATION OF THE CARNIVAL, BUT IN NO EVENT
FOR LONGER THAN 14 CONSECUTIVE DAYS.

(5)   A LICENSE TO CONDUCT A RAFFLE SHALL BE VALID
UNTIL THE TIME OF THE DRAWING OR DRAWINGS. ALL DRAWINGS FOR
A RAFFLE SHALL BE CONDUCTED WITHIN 1 CALENDAR DAY, THE DAY
FOR THE DRAWING OR DRAWINGS SHALL BE SPECIFIED ON THE
APPLICATION FOR THE LICENSE AND ON THE LICENSE.

(d)   Any organization [refusing to file the report set
forth in subparagraph (b) (4) forfeits it right to the
issuance of a license authorized by this section for a
period of one year] WHICH FAILS TO FILE A PROPER REPORT AS
REQUIRED BY THIS SECTION SHA1L FORFEIT ITS RIGHT TO THE
ISSUANCE OF ANY LICENSE AUTHORIZED BY THIS SECTION FOR A
