(4) WITH RESPECT TO ANY INTEREST ACQUIRED AFTER
APRIL 15, 1974:

(4) (I) THE DATE WHEN, THE MANNER IN WHICH, AND
THE IDENTITY OF THE PERSON FROM WHOM THE INTEREST WAS
ACQUIRED; AND

(5) (II) THE NATURE AND THE AMOUNT     OF THE

CONSIDERATION GIVEN IN EXCHANGE FOR THE INTEREST     OR, IF

ACQUIRED OTHER THAN BY PURCHASE, THE FAIR MARKET   VALUE OF
THE INTEREST AT THE TIME ACQUIRED.

(d) Gifts. — A schedule of each gift PERMITTED BY LAW
in excess of $50 $25 in value OR A SERIES OF GIFTS TOTALING
$200 $100 OR MORE FROM ANY ONE PERSON received at any time
during the year for which the statement is filed by the
person making the statement, or by any other person at the
direction of the person making the statement, from, or on
behalf of, directly or indirectly, any person who does
business with the State [or is regulated by the State,] or
is a [registered legislative agent] REGISTRANT; provided,

however, THAT neither . HOWEVER, gifts received from the

spouse or a relative within the third degree of

consanguinity of the person making the statement, or from

which are otherwise reported as required by law,—need A

MEMBER OF THE IMMEDIATE FAMILY OF THE PERSON MAKING THE

STATEMENT NEED NOT; PROVIDED, HOWEVER, THAT NEITHER GIFTS
RECEIVED FROM MEMBERS OF THE IMMEDIATE FAMILY, OTHER
CHILDREN, AND PARENTS OF THE PERSON MAKING THE STATEMENT NOR
CAMPAIGN CONTRIBUTIONS WHICH ARE OTHERWISE REPORTED BY NAME
OF DONOR NEED be disclosed. This schedule, as to each such
gift, shall include:

(1)   The nature and value of the gift; and

(2)   The identity of the person from whom, or on
behalf of whom, directly or indirectly, the gift was
received-

(e) Employment by entity doing business with State.
-—A schedule of all offices, directorships and salaried
employment held BY THE PERSON MAKING THE STATEMENT OR BY ANY
MEMBER OF HIS IMMEDIATE FAMILY at any time during the year
for which the statement is filed in any corporation or other
business entity which does business with the State. This
schedule, as to each such office, directorship or salaried
employment, shall include:

(1)   The name and address of the principal office
of the business entity;

(2)   The title and nature of the office,
directorship or salaried employment held AND THE DATE IT
COMMENCED; and
