AFFIDAVIT OR OTHER PROOF, SATISFACTORY TO THE

ADMINISTRATION, OF:

(I)    THE PRIOR NAME; AND

(II)   THE DIVORCE A VINCULO MATRIMONII.

SECTION 2. AND BE IT FURTHER ENACTED, That this Act
shall take effect July 1, 1976.
