[Ch. 568

(B)       EXCEPT AS MAY BE OTHERWISE PROVIDED BY THE
DEPARTMENT, AN ORIGINAL AND TWO CONFORMED COPIES OF EACH
PETITION FOR A TEMPORARY VARIANCE SHALL BE FILED WITH THE
DEPARTMENT AND SHALL INCLUDE OR BE ACCOMPANIED BY THE
FOLLOWING INFORMATION, TO BE VERIFIED BY AFFIDAVIT OF THE
PETITIONER:

(1)      THE NAME, ADDRESS AND TELEPHONE NUMBER
OF THE PETITIONER AND ANY OTHER PERSON AUTHORIZED TO
RECEIVE NOTICES ON THE PETITIONER'S BEHALF.

(2)      THE TYPE AND LOCATION OF THE OPERATIONS
GIVING RISE TO THE EMISSIONS FOR WHICH A VARIANCE IS
SOUGHT, INCLUDING A DESCRIPTION OF THE PROCESS GIVING RISE
TO THE EMISSIONS AND THE QUANTITY AND NATURE OF EXISTING
EMISSIONS.

(3)      A DESCRIPTION OF THE SPECIFIC VARIANCE
SOUGHT, INCLUDING THE COMMENCEMENT AND TERMINATION DATES
FOR WHICH THE VARIANCE IS SOUGHT AND THE TYPE OF FUEL,
ITS SULPHUR CONTENT AND THE QUANTITIES INVOLVED WHICH THE
PETITIONER PROPOSES TO UTILIZE.

(4)      A DESCRIPTION OF THE AMOUNT AND TYPE OF
THE PETITIONER'S CURRENT FUEL INVENTORIES AND OF ANY FUEL
RESERVES AVAILABLE TO THE PETITIONER AT ANY OTHER
FACILITIES OWNED OR OPERATED BY THE PETITIONER.

(5)     A STATEMENT OF THE REASONS WHY THE
PETITIONER IS UNABLE TO OBTAIN THE NECESSARY COMPLYING
FUEL. [[DOCUMENTATION SHALL INCLUDE:

(I)    A LIST OF SUPPLIERS, DOMESTIC AND
FOREIGN, FROM WHICH CONTRACTS WERE SOLICITED, INCLUDING
NAMES, DATES AND PLACES;

(II)   THE TYPES OF CONTRACTS WHICH WERE
SOLICITED, THEIR RESPECTIVE DURATIONS AND PRICES, AND THE
TYPES, QUALITY AND QUANTITY OF FUEL SOUGHT; AND

(III)   THE TYPES OF ANY CONTRACTS WHICH
WERE AVAILABLE, BUT WERE REJECTED BY THE PETITIONER.

(6)     A DESCRIPTION OF ENERGY CONSERVATION
MEASURES WHICH THE PETITIONER HAS ADOPTED OR PROPOSES TO
ADOPT AT ITS FACILITY.

(7)    AN ESTIMATION OF THE IMPACT WHICH THE
VARIANCE SOUGHT WILL HAVE ON AIR QUALITY.

(8)      WHATEVER ADDITIONAL INFORMATION THE
DEPARTMENT REQUESTS.]]

(C)    UPON THE FILING OF AN ACCEPTABLE VARIANCE
