616                                     LAWS OF MARYLAND                             Ch. 243

(B)  EACH INDIVIDUALIZED TREATMENT PLAN SHALL CONTAIN:

(1)   A STATEMENT DESCRIBING THE NATURE AND EXTENT OF
ANY MENTAL DISORDER OF THE PATIENT AND A DESCRIPTION OF
ANY MEDICAL CONDITION OR DISORDER WHICH MIGHT AFFECT
OR RELATE TO THE PATIENT'S COURSE OF TREATMENT OR
PROGNOSIS WHILE HE REMAINS A PATIENT; AND

(2)   A DESCRIPTION OF THE IMMEDIATE, INTERMEDIATE AND
LONG-RANGE TREATMENT GOALS, INCLUDING, BUT NOT LIMITED
TO, A PROJECTED TIMETABLE FOR THEIR ATTAINMENT; AND

(3)  A DESCRIPTION OF THE TREATMENT AND REHABILITATION
PLAN WHICH IS TO BE IMPLEMENTED FOR ACHIEVING THESE
IMMEDIATE, INTERMEDIATE AND LONG-RANGE GOALS,
INCLUDING, BUT NOT LIMITED TO, A PROJECTED TIMETABLE FOR
THEIR IMPLEMENTATION: AND

(4)   A STATEMENT OF THE LEAST RESTRICTIVE TREATMENT
CONDITIONS NECESSARY TO INSURE THE SAFETY OF THE
PATIENT OR OTHERS AND TO ACHIEVE THE GOALS OF
TREATMENT AND PURPOSES OF COMMITMENT; AND

(5)   A STATEMENT OF THE CRITERIA FOR MOVEMENT OF THE
PATIENT INTO EACH STAGE OF THE REHABILITATION PLAN; AND

(6)   A DESCRIPTION OF THE PROPOSED STAFF INVOLVEMENT
WITH THE PATIENT IN ORDER TO OBTAIN EACH OF THE
TREATMENT GOALS SPECIFIED; AND

(7)   A NOTATION AS TO ANY THERAPEUTIC TASKS, LABOR OR
WORK ASSIGNMENTS TO BE PERFORMED BY THE PATIENT,
INCLUDING A STATEMENT OF THE RATIONALE FOR THESE TASKS,
LABOR OR WORK ASSIGNMENTS AND ITS RELATIONSHIP TO THE
TREATMENT PLAN AND PROJECTED GOAL; AND

(8)     WHERE MADE APPLICABLE BY SUBSECTION (F), A
DESCRIPTION OF THE INDIVIDUALIZED EDUCATIONAL PLAN
WHICH HAS BEEN DEVELOPED, AND A DESCRIPTION OF
INTERMEDIATE AND LONG-RANGE EDUCATIONAL GOALS, WITH A
PROJECTED TIMETABLE FOR THEIR IMPLEMENTATION AND
ATTAINMENT; AND

(9)    A PROJECTION OF THE EARLIEST POSSIBLE DATE OF
DISCHARGE, INCLUDING QUALIFYING REMARKS REGARDING THE
PATIENT'S PROGRESS AS ARE APPROPRIATE.

(C)     IN THE INTERESTS OF CONTINUITY OF CARE, ONE
QUALIFIED MENTAL HEALTH PROFESSIONAL SHALL BE
DESIGNATED BY THE SUPERINTENDENT TO BE RESPONSIBLE FOR
SUPERVISING THE IMPLEMENTATION OF THE TREATMENT PLAN,
INTEGRATING THE VARIOUS ASPECTS OF THE TREATMENT
PROGRAM AND RECORDING THE PATIENT'S PROGRESS. IT SHALL
BE THE RESPONSIBILITY OF THIS QUALIFIED MENTAL HEALTH
PROFESSIONAL TO INSURE THAT THE PATIENT IS TRANSFERRED,
WHEN APPROPRIATE, INTO A LESS RESTRICTIVE FORM OF
TREATMENT, AND TO INSURE THAT, WHEN APPROPRIATE,
RECOMMENDATIONS ARE FORWARDED TO THE SUPERINTENDENT
FOR THE PATIENT'S TOTAL OR CONDITIONAL RELEASE FROM THE
