TICKETS REDEEMABLE IN CASH OR MERCHANDISE, OR
ANY OTHER THING OF VALUE AS A PRIZE OR AWARD TO
THE PATRONS OF SUCH LICENSED GAME; PROVIDED, HOW-
EVER, THAT NO PRIZE OR AWARD HAVING A VALUE OF
MORE THAN ONE HUNDRED DOLLARS SHALL BE ISSUED
ON ANY ONE GAME, EXCEPT THAT ONCE EACH DAY A
GRAND PRIZE OF NOT EXCEEDING ONE THOUSAND FIVE
HUNDRED DOLLARS AND ONE GRAND PRIZE OF NOT EX-
CEEDING THREE THOUSAND DOLLARS IN RETAIL VALUE
MAY BE ISSUED.

(3)  ALL APPLICATIONS FOR THE ISSUANCE OF SUCH
LICENSES SHALL BE FILED NOT LESS THAN THIRTY
DAYS PRIOR TO THE ISSUANCE OF SUCH LICENSE BY
THE COUNTY COMMISSIONERS OF CALVERT COUNTY,
WHICH APPLICATION SHALL SET FORTH:

(I)  THE NAME AND ADDRESS OF THE APPLICANT, AND
TRADE NAME, IF ANY.

(II)   IF AN INDIVIDUAL OR PARTNERSHIP, THE NAMES
AND ADDRESSES OF THE INDIVIDUALS OR PARTNERS.

(III)  IF THE APPLICANT IS A CORPORATION, THE
NAMES AND ADDRESSES OF ALL THE OFFICERS OF THE
CORPORATION.

(IV)  THE NAME AND ADDRESS OF THE APPLICANT'S
RESIDENT AGENT, IF A RESIDENT AGENT IS REQUIRED.

(V)  THE NAMES AND ADDRESSES OF ALL PERSONS
HAVING A FINANCIAL INTEREST IN THE OPERATION OF
THE PROPOSED GAME.

(VI)  ALL PERSONS LISTED IN (I) TO (V) ABOVE SHALL
AFFIX THEIR SIGNATURE TO THE LICENSE APPLICATION
AND SHALL CONSENT TO INDIVIDUAL LIABILITY FOR
ANY VIOLATION OF THE LAWS IN THE OPERATION OF
SUCH LICENSED GAME.

(4)  A CLASS ND OR CLASS NE BEACH BINGO LICENSE
SHALL PERMIT THE OPERATION OF SUCH LICENSED
GAME FROM MAY FIRST UNTIL SEPTEMBER THIRTIETH
IN ANY ONE YEAR ONLY. EACH APPLICANT SHALL PRE-
SENT EVIDENCE TO THE COUNTY COMMISSIONERS OF
CALVERT COUNTY THAT HE HAS IN EFFECT FOR THE
PERIOD COVERED BY THE PROPOSED LICENSE A PUBLIC
LIABILITY INSURANCE POLICY COVERING LIABILITY FOR
PERSONAL INJURY TO ANY ONE PATRON OF SUCH GAME
IN THE AMOUNT OF NOT LESS THAN ONE HUNDRED
THOUSAND DOLLARS AND FOR MORE THAN ONE PATRON
IN THE AMOUNT OF FIVE HUNDRED THOUSAND DOLLARS.

(5)  EACH NON-RESIDENT APPLICANT FOR A LICENSE
SHALL DESIGNATE A RESIDENT AGENT WHO SHALL BE
A VOTER AND A TAXPAYER OF THE COUNTY OWNING
PROPERTY IN THE COUNTY ASSESSED AT NOT LESS THAN
FIVE THOUSAND DOLLARS.

(6)  NO LICENSE SHALL BE TRANSFERABLE. NO LICENSE
SHALL BE ISSUED IF THE OPERATION OF ANY BINGO
