AND/OR MODIFICATIONS TO THE DIRECTOR. IF THE
CHANGES ARE CORRECTED THE DIRECTOR SHALL BE
AUTHORIZED TO ISSUE THE PERMIT. IF THE COMMIT-
TEE REJECTS THE PLAN THE DIRECTOR SHALL NOT
APPROVE THE APPLICATION INVOLVED.

664.

(a)    On or before the 25th day of each month, every operator of a
coal stripping operation shall furnish the Bureau of Mines, on a form
provided by the Bureau, [an accurate] a monthly report of coal
produced, number of employees and days worked, and [also] a report
of all fatal and nonfatal accidents for the previous month. An
annual report shall be furnished to the Bureau not later than the
twenty-fifth day of [January] February each year for the preced-
ing year covering facts regarding the production and condition of
the operation during the calendar year preceding as the Bureau may
require.

(b)    [(c)] If an operator continues to operate beyond one year,
the Director shall be furnished with a new map at the end of each
year of operation, on which is outlined the area affected and progress
of reclamation during the preceding year. This Mining and
Reclamation Progress Report shall contain information identical
to that required for both the reclamation plan and the final rec-
lamation report and shall show in detail completed reclamation work.
Failure of the Committee to act upon a reclamation progress report
within sixty (60) days after its submission shall constitute approval
of the report. If no land has been disturbed by operations during
the preceding year, the operator shall notify the Director of this
fact.

(c)    [(b)] [Any mine inspector directed by the Bureau of
Mines] The Director or his agent may enter upon and inspect
[all] any open-pit operations for the purpose of determining con-
ditions of safety and for compliance with the provisions of this sub-
heading and all rules and regulations promulgated pursuant thereto.
If an operator fails to comply with the requirements of this sub-
heading, or any rules or regulations promulgated pursuant thereto,
[the mine inspector shall report the matter to] the Director [,
who] shall immediately notify the operator by registered mail or by
personal delivery by his agent
of such failure and [requiring]
require compliance within a specified time. Unless the operator
complies with the subheading and such rules and regulations within
the time specified, the Director [, after hearing and final deter-
mination,] may [cancel the permit on this operation and/or]
suspend the open pit mining operator's [license] permit, and may
issue a cease and desist order requiring the operator immediately to
cease open-pit mining ON THE OPERATION IN QUESTION within
this State until it is determined by the Director that the operator is
