he shall apply to the Board of said county or legislative district
BALTIMORE CITY to which he has removed, for registration. He
shall be registered in the manner provided in this Article, and shall
state the fact of his removal from such other county or legislative
district
BALTIMORE CITY. The voter shall sign and acknowledge
before a person duly authorized to administer oaths,
INCLUDING
EMPLOYEES OF THE BOARD OF SUPERVISORS AUTHOR-
IZED BY THE BOARD TO DO SO, an authorization to cancel his
previous registration on a form substantially as follows:

"CANCELLATION AUTHORIZATION

I hereby authorize the cancellation of my previous registration
in the State of Maryland, in the County of
............................, ELEC-
TION DISTRICT ........................, PRECINCT ........................, or the

legislative district of Baltimore City, Ward ........................, Precinct

........................................ My last registration address therein being

........................................ AND MY PREVIOUS PARTY AFFILIA-
TION BEING........................................MY PRESENT ADDRESS IS

........................................, COUNTY.........................., ELECTION DIS-
TRICT .........................., PRECINCT........................

authorized to administer oaths)"

The Clerk of the Board of Supervisors of Elections shall mail all
such authorizations to the Board of Supervisors of Elections of the
proper county or legislative district
BALTIMORE CITY. Upon
receipt of this authorization from the forwarding county or legis-
lative district
BALTIMORE CITY, the Clerk of the Board of Election
Supervisors upon a comparison of the voter's signature with his
signature as it appears on the registration records shall remove the
voter's registration from the records and retain the cancellation
authorization in the records of said Board. The Board of Super-
visors shall notify the voter at
BOTH the present AND LAST REGIS-
TRATION address as shown on the cancellation authorization that
his previous registration has been cancelled.

CANCELLATION OF REGISTRATION

43. Reports to the Boards. (a) Persons Deceased. The Commis-
sioner of Health of Baltimore City and the Health Officers of the
several counties shall file with the Board of Supervisors of Elections
of their respective city or counties, as the case may be, a list of
the names and residence addresses (if known) of all persons over
