one copy shall be retained in the files of the Department for
a period of four years from date of issue. All certificates
issued in any of the Counties of Maryland shall be made out
in duplicate and one copy shall be delivered by the person
issuing the certificate to the Department of Labor and In-
dustry and shall be preserved in the files of the- Department
for the period of four years from the date of issue; and the
person issuing the certificates in any of the counties shall
also make a record of each application for any employment
certificate upon blanks furnished by the Department and
shall preserve same for a period of four years from the date
of application. All employment certificates shall also contain
the name and address of the prospective employer and the
nature of the occupation in which the child is to be engaged.
No certificate shall be valid except in the hands of the em-
ployer so named and for the occupation so described.

(i) The officer authorized under this article to issue em-
ployment certificates shall upon application by any minor
over 18 years of age or by any employer for the minor, issue
to such minor or employer a certificate of age card, certifying
the age of such minor, upon receiving, examining and approv-
ing proofs and evidence of age of the same type and in the
same order as required under Section 21 24 of this sub-title for
the issuance of employment certificates.

Such certificates of age cards shall be issued on forms pre-
pared and supplied by the Department of Labor and Industry
and shall contain the name, date of birth and place of resi-
dence of the minor for whom issued and such other informa-
tion as the Department shall deem pertinent.

(j) Upon the termination of the employment of any minor
to whom such a certificate of age card has been issued, the
employer shall immediately, and in no case longer than 24
hours thereafter, return to such minor any certificate of age
card which may have been turned over to the employer of
such minor.

(k) Any minor who with intent to deceive, uses as if it
were issued to him a certificate of age card issued to another
minor and any minor, or parent, guardian or custodian of
any minor, or any other person, who with intent to deceive,
changes or alters, or aids in changing or altering any birth
or other record submitted as proof of age to obtain a cer-
tificate of age card or an employment certificate shall be
guilty of a misdemeanor and upon conviction shall be pun-
ished for each offense by a fine of not more than twenty-five
dollars.
