We, the undersigned, qualified electors request the candi-
dacy of

Name. Street. Ward.

Immediately upon the expiration of the time for filing cer-
tificates of nomination the town Clerk shall make up the official
Ballot from the names so submitted and shall cause to be pub-
lished, in one or more newspapers printed or circulated in said
town, a fac simile thereof authenticated by his signature.
The official Ballot shall be of substantial white paper not less
than four or more than six inches wide, and there shall be a
square at the right of the name of each candidate and a blank
space at the end of the list of candidates for each office, with
a square opposite said blank space, so that any voter may write
therein the name of any person for whom he may wish to vote
for said office whose name shall not appear upon the official
Ballot. In order to vote for any candidate or candidates the
voter shall make a cross (X) in the square opposite the name
of each candidate for whom he may wish to vote. The official
Ballot shall be as nearly as practicable in accordance with the
following form, the date and names to be changed but the
names of all candidates for Mayor to be arranged alphabeti-
cally and the wards arranged in their numerical order, and
the names of all councilmen in each ward to be arranged alpha-
betically.
