tion and residence of the person or persons so affected, together

 

with such other information as may seem necessary or import-

 

ant. And all such records shall be delivered, under seal, to the

 

State Board of Health, on Monday of the week immediately

 

following that in which the records were made. Any superin-

 

tendent or other person charged with a duty under this section,

 

who shall fail or refuse to comply with the requirements of

 

this section, shall be deemed guilty of a misdemeanor, and on

 

conviction thereof shall be fined not more than twenty-five
