The Long Term Care Division directs the plan-
ning, implementation, and evaluation of long term
care services for the elderly managed by the Office
on Aging in collaboration with other State depart-
ments and agencies. The Division develops poli-
cies and initiatives for promoting statewide long
term care activities aimed at reducing the institu-
tionalization of the frail elderly. The division pro-
vides staff support to the Interagency Committee
on Aging Services. The Division also directs the
Ombudsman, Public Guardianship, and Life Sup-
port programs, in addition to Gateway II. Gate-
way II is a system operated at the local level to
provide screening, assessment, and case manage-
ment services to frail elderly persons.
The Housing and Continuing Care Division de-
velops, certifies, and assists in the operation of
subsidized sheltered housing for the elderly. Such
housing provides a residential environment with
assistance as needed for activities of daily living.
The Division also regulates and certifies continu-
ing care facilities operated by the private sector to
provide long-term shelter and care to elderly per-
sons. Information disclosed through the registra-
tion process is open to public inspection as a con-
sumer protection to prospective residents.
The Planning and Evaluation Division is re-
sponsible for program planning and development,
establishment of program priorities, and assess-
ment of program results. It directs and manages
legislative, research, health insurance, and other
Office activities that benefit consumers.
