(ii) the death of or personal injury to a qualified person;

(2)     (i) at the time of the accident, the claimant was not driving or
riding in an uninsured motor vehicle owned by the claimant or a member of the
claimant's family who resides in the claimant's household; and

(ii) the claimant is not the personal representative of the individual
who was driving or riding in the uninsured motor vehicle; and

(3)     (i) at the time of the accident, the claimant was not driving a motor
vehicle with a certificate of registration that was suspended, canceled, or revoked, or
was holding a driver's license that was suspended, canceled, or revoked; and

(ii) the claimant is not the personal representative or a member of
the family who resides in the household of the individual who was driving the motor
vehicle.

(b) A premium finance agreement shall contain:

(5) if applicable, the following items:

(iii) the principal balance (the difference between [subitems] ITEMS
(i) and (ii) of this item);

(v) the balance payable by the insured (the sum of [subitems]
ITEMS (iii) and (iv) of this item);

On approval of the application for a certificate of authority, the Commissioner
shall issue to the Society a certificate of authority that authorizes the Society to issue:

(1) the following policies of casualty insurance:

(i) insurance against the liability of physicians or other health care
providers for injury arising out of the rendering of or failure to render professional
services by the insured;

(ii) insurance against the liability of a person for whose acts or
omissions a physician or other health care provider is responsible under [subitem]
