Ch. 357 2003 LAWS OF MARYLAND 14-109. An applicant for a certificate of authority shall: (1) file with the Commissioner an application on the form that the (2) pay to the Commissioner the applicable fee required by § 2-112 of this (3) file with the Commissioner copies of the following documents, certified (i) articles of incorporation, INCLUDING THE APPLICANT'S (ii) bylaws with all amendments; (iii) each contract executed or proposed to be executed by the (iv) each form of contract issued or proposed to be issued to (v) a financial statement of the corporation, including the amount of (vi) a list of the names and addresses of and biographical (VII) A LIST OF THE TOTAL COMPENSATION PAID OR PROPOSED TO (VIII) A LIST OF THE BEGINNING AND ENDING TERMS OF [(vii)](IX) any other information or documents that the Commissioner 14-110.
