Ch. 237 2003 LAWS OF MARYLAND (2) HOLD AN ACTIVE LICENSE TO PRACTICE DENTAL HYGIENE IN (3) COMPLETE THE CONTINUING EDUCATION REQUIREMENTS THAT (4) PROVIDE DENTAL HYGIENE SERVICES EXCLUSIVELY IN THE (5) IMMEDIATELY UPON CEASING TO PROVIDE SERVICES EXCLUSIVELY 4-304. (c) To apply for a (1) Shall submit an application to the Board on the form that the Board (2) May not be required to pay an application fee.
