MEDICAL BENEFITS.

(D) (1) IF A HEALTH INSURANCE POLICY ISSUED BY AN INSURER CONTAINS
A NONDUPLICATION PROVISION OR A PROVISION TO COORDINATE THE COVERAGE
WITH OTHER HEALTH INSURANCE POLICIES, THE INSURER SHALL USE THE
UNIFORM COORDINATION OF BENEFITS FORM ADOPTED BY THE COMMISSIONER
UNDER § 490BB OF THIS ARTICLE AS THE SOLE INSTRUMENT FOR DETERMINING
NONDUPLICATION OR COORDINATION OF COVERAGE WITH OTHER HEALTH
INSURANCE POLICIES UNDER WHICH THE INSURED MAY MAKE. A CLAIM.
(2) AN INSURER MAY NOT IMPOSE AS A CONDITION OF
REIMBURSEMENT ANY REQUIREMENT TO;
(I) MODIFY THE UNIFORM COORDINATION OF BENEFITS FORM;
OR
(II) SUBMIT ADDITIONAL COORDINATION OF BENEFITS FORMS.

(C) EVERY INSURER SHALL USE THE UNIFORM SUMMARY EXPLANATION OF
BENEFITS FORM ADOPTED BY THE COMMISSIONER UNDER § 490BB OF THIS ARTICLE
AS THE SOLE INSTRUMENT FOR PROVIDING THE SUMMARY EXPLANATION OF
BENEFITS TO AN INSURED UNDER THIS SECTION.

(H) (1) EVERY INSURER SHALL USE THE UNIFORM PROVIDER VOUCHER
FORM ADOPTED BY THE COMMISSION UNDER § 490BB OF THIS ARTICLE AS THE SOLE
INSTRUMENT FOR PROVIDING THE EXPLANATION OF PAYMENT FOR MEDICAL
BENEFITS TO A PROVIDER;
(2) AN INSURER MAY NOT:
(I) MODIFY THE UNIFORM PROVIDER VOUCHER FORM; OR
(II) PROVIDE ADDITIONAL FORMS FOR THE EXPLANATION OF
MEDICAL BENEFITS.

(A) (1) IF AN INSURER REQUIRES ITS INSUREDS TO HAVE A REFERRAL TO
RECEIVE LABORATORY SERVICES IN WRITING, THE INSURER SHALL USE THE
UNIFORM LABORATORY REFERRAL FORM ADOPTED BY THE COMMISSIONER
UNDER § 490BB OF THIS ARTICLE AS THE SOLE INSTRUMENT FOR REFERRALS FOR
LABORATORY SERVICES.
(2) AN INSURER MAY NOT IMPOSE AS A CONDITION OF COVERAGE ANY
REQUIREMENT TO;
(I) MODIFY THE UNIFORM LABORATORY REFERRAL FORM; OR
