(v) the name and address of the representative member, if the licensee
is a firm.

(2) The [Superintendent] SECRETARY shall include on each branch office
certificate:

(i) the full name of the licensee;

(ii) the location of the principal office of the licensee and of the
agency branch office for which the certificate is issued;

(iii) the date of issuance of the branch office certificate;

(iv) the date on which the license of the licensee expires; and

(v) the name and address of the representative member, if the licensee
is a firm.

DRAFTER'S NOTE:

This section is amended to reflect the status of the head of the Department of
State Police as the Secretary of a principal department of State government in
light of Chs. 165 and 166, Acts of 1994.

13-308.

(b)     At least 1 month before a license expires, the [Superintendent] SECRETARY
shall mail to the licensee, at the last known address of the licensee:

(1)     a renewal application form; and

(2)     a notice that states:

(i) the date on which the current license expires;

(ii) that the [Superintendent] SECRETARY must receive the renewal
application and the annual statements required under § 13-309 of this subtitle, on or
before April 15, for the renewal to be issued and mailed before the license expires;

(iii) the amount of the renewal fee;

(iv) that, if the annual statements are not received on or before April
15, a fee of $10 per day shall be charged against the licensee until the statements are
received; and

(v) that the submission of a false statement in the renewal application
or in the annual statements is cause for revocation of the license.

(c)     A licensee periodically may renew the license for an additional 1-year term, if
the licensee:

(1)     otherwise is entitled to be licensed;

(2)     pays to the [Superintendent] SECRETARY:
