GLENDENING, Governor Ch. 97 (f) Each clerk of court shall send to the Secretary, on the form that the Secretary (1) Each decree of adoption; (2) Each adjudication of paternity, including the father's Social Security (3) Each revocation or amendment of any decree of adoption or (G) UPON RECEIPT OF A REPORT OR DECREE OF ANNULMENT OF ADOPTION, (H) IF NO CERTIFICATE OF BIRTH IS ON FILE FOR THE PERSON FOR WHOM A (1) A DELAYED CERTIFICATE OF BIRTH SHALL BE FILED WITH THE (2) THE NEW BIRTH CERTIFICATE SHALL BE PREPARED ON THE (I) (1) THE SECRETARY SHALL, UPON REQUEST, PREPARE AND REGISTER (2) THE CERTIFICATE SHALL BE ESTABLISHED UPON RECEIPT OF A (3) THE CERTIFICATE SHALL BE LABELED "CERTIFICATE OF FOREIGN (4) A STATEMENT SHALL ALSO BE INCLUDED ON THE CERTIFICATE 4-212. (a) This section does not apply to a fetal death. (b) (1) A certificate of death REGARDLESS OF AGE OF DECEDENT shall be - 1447 -
