UNSATISFACTORY WORK OR CONDUCT REPORT. (A) IN GENERAL. (1) EACH APPOINTING AUTHORITY SHALL SUBMIT A REPORT TO THE (2) THE SECRETARY SHALL DETERMINE THE FORM OF THE REPORT. (B) NOTICE TO EMPLOYEES. THE APPOINTING AUTHORITY SHALL: (1) PROVIDE A COPY OF THE REPORT TO THE EMPLOYEE; AND (2) NOTIFY THE EMPLOYEE OF THE EMPLOYEE'S RIGHT TO RESPOND (C) EMPLOYEE RESPONSE.
