(C) WITHIN 30 DAYS AFTER A PARENT IS ORDERED TO OBTAIN HEALTH
INSURANCE UNDER THIS SECTION, THE PARENT SHALL PROVIDE TO THE SUPPORT
ENFORCEMENT AGENCY AND THE OTHER PARENT:
(1) THE PARENT'S SOCIAL SECURITY NUMBER;
(2) THE NAME AND ADDRESS OF THE PARENT'S EMPLOYER;
(3) WHETHER THE PARENT'S EMPLOYER HAS HEALTH INSURANCE
AVAILABLE THAT CAN PROVIDE COVERAGE FOR THE CHILD;
(4) PROOF THAT HEALTH INSURANCE HAS BEEN PROVIDED FOR THE
CHILD; AND
(5) ANY OTHER INFORMATION NECESSARY TO MAKE A CLAIM,
INCLUDING:
(I) THE NAME OF THE HEALTH INSURANCE CARRIER;
(II) THE POLICY NUMBER;
(III) A SCHEDULE OF BENEFITS;
(IV) A MEMBERSHIP CARD; AND
(V) CLAIM FORMS.
(D) (1) IF A COURT ORDERS A PARENT TO PROVIDE HEALTH INSURANCE
UNDER THIS SECTION, A PARENT OR THE SUPPORT ENFORCEMENT AGENCY MAY
SEND A CERTIFIED COPY OF THE ORDER TO THE EMPLOYER BY CERTIFIED MAIL.
(2) WITHIN 30 DAYS AFTER THE EMPLOYER RECEIVES NOTICE OF A
COURT ORDER REQUIRING HEALTH INSURANCE, THE EMPLOYER SHALL:
(I) ENROLL THE CHILD IN A HEALTH INSURANCE PLAN
AVAILABLE TO THE PARENT;
(II) PROVIDE A STATEMENT TO THE SUPPORT ENFORCEMENT
AGENCY AND THE PARENT THAT THE CHILD:
1. HAS BEEN ENROLLED IN A HEALTH INSURANCE PLAN;
2. WILL BE ENROLLED IN A HEALTH INSURANCE PLAN AT
THE NEXT AVAILABLE ENROLLMENT PERIOD AND PROVIDE THE EXPECTED DATE
OF ENROLLMENT; OR
3. CANNOT BE ENROLLED IN A HEALTH INSURANCE PLAN;
AND
(III) PROVIDE INFORMATION CONCERNING THE AVAILABLE
HEALTH INSURANCE COVERAGE, INCLUDING:
1. THE NAME OF THE HEALTH INSURANCE CARRIER;
2. THE POLICY NUMBER;
